They don't. I don't experience sadism to any degree, and rarely masochism. When I hear about someone abusing a child in that way it just makes me very angry. I couldn't imagine finding the child's pain or humiliation arousing.

Never. Mostly I just imagine the way they look. The older the girl is (by which I mean the more sexually aware she is), the more physical intimacy may feature in the fantasy, and then it's imagined as mutual and thoroughly enjoyed.

Sometimes.

I like to imagine tears, whimpers and such like. I also find the fantasy of soothing them whilst at the same time hurting them plays a part in my imagination. But that's only sometimes, not all the time. I haven't been able to correlate it with certain emotions that I'm feeling at the time, there doesn't seem to be a connection.

May I ask why you ask such question?
