So apparently a diagnostic criterion of OCD is to know that your fears are irrational. But some people do not know their fears are irrational.
I wonder how this is for you? I'm definitely the type for who it's not irrational and I cannot distinguish between truth and lie and think my TOCD is real. Like, it feels real. I'm doubting and I wonder if I'm not actually Trans.
How is that for you?
The lost look a lot like me. In dust I was born and dust I shall leave.

Lisa, the DSM-V REMOVED that line for criteria of diagnosis.
I think you know my answer on how it is for me.

At the very beginning I knew it was an irrational fear. I had thoughts like, "Oh my god, why I can't I stop thinking such stupid stuff?!" but the more I was checking for attraction and arousal, looking at "signs" from the past the more I felt like it was real (I got "proof" like groinal responses etc.).
