This is what I've been doing on a spreadsheet (following the same layout of a printed page a doctor gave me). "D" means "Down" (going to bed), "U" is "up" for waking up. Solid lines are sleep, short/broken ones for when I woke up. Each box is a 15-min span (but that isn't needed really). The last number "sleep quality" is how well I feel in the morning, medium (5-7) is about average MOST days.
It also has E, C, and A for Exercise, Caffeine, and Alcohol (something the doctor had me tracking). But a simplified thing would be much better since this level of tracking isn't needed anymore (sleep is, but the rest isn't), but I've just kept doing what I was always doing b/c it was what I had started and ready to go.
The "444444" and "555555" are incidents where I blacked out or spaced out, long enough to cover the entire time span of the incident (details for each incident (always numbered) are in footnotes below the table).
https://imgur.com/a/eGzEZWPI will add, too, that your version is much simpler, cleaner, and looks a LOT easier to keep up with. I may absolutely adopt that (or something very similar to it).
