I know this is an old thread but I wanted to respond anyway... I hope that's OK.

Like all of a sudden I'm there again, I feel "real" and not foggy again, things seem more clear, etc. Except it doesn't just happen in the car, it can happen anywhere. Not sure if that counts.

Something like that happens to me too. I have had this my whole life, a sudden moment of realizing who I am and what is going on. Like a sudden moment of seeing the "bigger picture" (often accompanied with thoughts about life etc.). But I'm usually able to remember what happened just before it.
This happens often when waking up from highway hypnosis (or more than that?), then I don't know what happened before but I have a sense of the passing time (so it's not literally lost time). I know that I'm not really away in the meantime, more like, deep in thought. Sometimes, when underway, I look back (literally) and think, whoa, what happened just yet? 'Cause it really feels like a sudden wake up.
Dx: PDD-NOS. Tested for dissociative disorders and PTSD but they say the symptoms are attributable to PDD-NOS.
