Survey on Self Esteem for Any and All Disorders/Ilness

My name is Aspen, I'm taking a Research Methods class at my community college. I have had to come up with a research proposal, and I'm now onto collecting data.

My research is to see what kind of affect having a mental illness/ psychological disorder has on a persons self esteem.

Take it easy Aspen... ...you've already created a thread about this. One should be enough, but I can see that this thread includes a link to the offline version of the survey. I will post a link in your other thread to link it to this one. There's no need for any more new threads though.
