Self-harm is always considered distress.
Other possibilities are not having a job or relationships. Those are all considered distress, whether you like it or not. It is factual, and not a subjective feeling.
Maybe I don't want friends, but the simple fact that I don't have any can be considered distress, because I lack a social network of support. Same for a job and so on. They are objective distress that have nothing to do with my feelings.

If I go and tell that to a doctor, they will be distressed for me.

I suppose having a long-term relationship that you do not want could also be considered distress. It sounds like it anyway. You want out but you don't, seems like a problem.
