When sorry isn't enough

I say I am sorry for doing wrong but then I do wrong again and by the people who don't believe I am sorry I show completelt no remorse for my actions and behaviors. I would laugh at things that won't be funny to other people. They think I am some psycho psychotic jerk. And I don't feel remorse but I say sorry to get out of trouble.When I get out of trouble I do it again. Then it goes back to the I am sorry stage and keeps repeating it over and over again. I have poor impulse control and with anger issues really bad

Have you ever spoken to a Dr or a therapist about your thought patterns re saying sorry esp to keep out of trouble? Also the impulse control and anger issues? It sounds like it could be a good idea to do so
