I'm wondering if this is something that happens in pure o patients or in general even? I was in the hospital and diagnosed with what the doctor called serious OCD. So one of my pills I'm on is an anti-psychotic and it has me wondering if I was delusional at that point. The thoughts I was having were of harming my family and they felt very possible at the time. So I'm wondering if the doctor thinks I'm having delusions or voices in my head telling me to do these things, I don't hear audible voices they are all in my brain and feel intrusive.
I'm not sure if this is the right section.

The idea of delusions and OCD is a tricky one. Because we are pounded with irrational thoughts, we often confuse if we really believe these thoughts or not. I have Bipolar Disorder, so the concept of delusions becomes even more confusing.
At any rate, there are aspects of your post I am surprised your doctor didn't go over, like asking you about the nature of your thoughts (audible voices or not, etc). Anti-psychotics have "off label" uses, so I don't want to presume what you are taking them for.
But the crux of your post is something that you should go over with your doctor - for a more clear idea of what he/she feels about your condition. Your questions are valid and should be answered. It's not wrong to discuss it here, but I think you would be better served by first getting your doctor's opinion since they are the one who diagnosed you and are treating you.
good luck, Otter.
