I was a highly functional NPD. I did well on exams and the SATs because I got narcissistic rage when others called me stupid. It was like a revenge mechanism to prove others wrong and because of my extreme fear for failure. I ended up at a top university, got a top end job, and frauded myself through life (likely didn't fraud through life but that is what my mentality thinks).
You may want to check if your NPD is comorbid with something else, or if you are suffering from addiction to something that brings you down (drugs, alcohol, etc.). Fortunately, my addiction was to challenges and puzzles (women, rubik's cubes, video games, etc.). Things that boosted my fake self-esteem to do better. Drugs, on the other hand, physically harm you and financially, which is a double whammy for your success potential.
