can anyone help

My boyfriend of 20 years, who is in his 40s, first started to expeirence symphoms around his 40s birthday. Many things happened all at once: he went of a high-protein (sever) diet, takes self-treating a yeast infection with high oral dosages of lamiil (purchased in mexico), got involved in a major lawsuit, received a death threat, and lastly was a witness to a major crime that invilved a high profile figure. His mother I am sure suffered from the illness, but he refuses to believe it. And he has gone from thinking his friends and family were betraying him to now it is the government torturing hims, so much so he has moved to Canada. And of course is having problems there. In these 4 years, he has lost his mother, unclude and grandmother (and of curse, feels the governement had a hand in their deaths). He never exhibited any of these problems befire and was a high-functiongperson prior. I know they say the disease tends to affect people in their 20s, but can someone 40 have it tigereged by a chemicla imbalanced caused by medications and diet? I can not get him to seek help. I am afraid he might take his own life.Any advise.
