Should I go back to the psych ward to complete the form?

I had to go to the psych ward because I was hysterical, I was crying a lot. At the psych ward, they asked me to sign my names on the forms.

But on one form, I forgot to fill out the form completely. I was supposed to check a box from the two boxes on the form. I didn't do a check because I had trouble making a proper check mark.

They discharged me. When I got home, I remembered that I did not fill out the form completely nor properly. This is making me nervous and I want to complete the form.

What should I do?

If I go back to the psych ward and ask to complete the form, will they try to make me live in the hospital? I don't want to live in the hospital. I only want to complete the form and go home. What should I do?

But if I don't complete the form properly and completely, I will feel nervous. What should I do?

What I would do in this situation is call the hospital and ask them what to do about it. That way, if they want to put you in the psych ward for some time, they really can't. Just say, "Hello, I was in the other day (or whenever), and I was discharged not too long ago, and I forgot to complete one of the forms you guys had me fill out. It's making me really anxious and everything. Is it a big deal?"

Something like that. If they ask for your name, give it to them. even give more information if you're comfortable with it. Honestly, they're probably not going to make a bid deal out of it, but if it helps you from being nervous, give them a call. Don't know their number? Google the hospital.
