Yes. This is a classic and common false dilemma, one of many: "am I ill or am I faking?" "Are they suicidal or are they faking?" "Do I love this person or do I love that person?"
An important technique of psychotherapy is uncovering such false dilemmas, so that the client can examine them closely and realize they are false. Voila, resolution!
