
A relative experienced a psychotic break 2/06, was hospitalized 2 wks, put on risperdol. About 3 wks later, experienced some very distressing news which sent her into a deep deep depression, was started on Paxil which has been titrated up to Paxil CR 50mg x4-5wks as of now. Although functional in the past has a very tragic history of 2 abusive marriages, a son w/alcholism with a h/o 2 suicide attempts and then finally succeeded in a 3rd suicide attempt a few yrs back, but left 3 young children behind and a wife.
Although in CBT and seeing psych MD every 4-6wks, she seems to have plateaued the past 2 mos. Initially was extremely negative, anxious, hopeless, worthless,etc. Now not as bad, but not good, no motivation unless someone is guiding/helping her, still negative, helpless, easily overwhelmed, remains anxious, but not as bad, etc. On a scale of 1-10 with 10 being fantastic, I would say she is a 2 or 3.
Is it time to think about a medication adjustment?
Is there anything else that might be done to help her, she is very frustrated, says "I can't stand it anymore." My fear is that if she doesn't convey this strongly to her therapist/Doc and some other options are considered to help her now-she may consider ending it all at some point if she doesn't see any light at the end of the tunnel.
Unfortunately, she has no relatives that live close to her, I am 3+hrs away, but thank heavens she seems to have made friends with a woman that tries to help her as best she can, however, she also lives and is treating for depression.
Any suggestions would be appreciated.
