Help with job application for school UK

Hi all, I need some help I am getting a job through an agency to work in schools, on the health questionnaire there's this question;

'Are you currently suffering from any mental illness, psychological or psychiatric problem, including depression, stress, anxiety, nervous debility, nervous breakdown, schizophrenia or eating disorder (anorexia or bulimia) that requires medical intervention?'

I am not on medication, I am getting counseling but that's it, should I/do I have to put my diagnosis down? The thing is I've worked for another branch of this agency before and my contact there knows I've got it due to a workplace issue and the occupational therapist they outsource this too properly already know, I should just put it down shouldn't I?
