I kind of think Narcissists have a natural empathy for other narcissists (meaning they understand, not that they necessarily care...). It's simply based on the fact that if somebody else perceived the world same as you, then whatever you would feel is likely to also be what they feel.

Our lack of empathy for regular people is partially based on misunderstanding as anything, you don't feel the same as what we would feel in similar circumstances so of course the empathy seems inappropriate or missing. Things bother you that our own defense mechanisms would shoot from the sky, so we have no clue why you're upset.
