A window will pop up telling you that the original document has been deleted by the owner. You click on "create copy". Firefox blocks the new doc on my computer, so you may have to click on "allow pop up" additionally. Et voila now you have the PID-5 on Google docs. You only have to fill in the numbers and hit the "results" button.
