The medical literature has several case reports of psychosis (including hallucinations) caused by prescribed doses of Adderall (and Dexedrine). This is a pretty rare reaction, but it is not unheard of -- except, I guess, by your health care providers.
For example, an article titled "Adderall-induced psychosis in an adolescent" in the Journal of the American Board of Family Practice can be obtained for free (in Adobe Acrobat format) at
http://www.jabfp.org/cgi/reprint/15/6/498
I found this through PubMed, a great database of biomedical journal articles, at

You might want to share that article with this psychiatrist or anyone else who isn't familiar with this phenomena. In most people, it does take a very large dose of amphetamines (which Adderall is) for psychosis to occur, but apparently in a few people small doses can do it.
Unfortunately all medications have a risk of side effects, and their efficacy varies between people. But you might want to ask your psychiatrist about non-amphetamine/non-stimulant medication like Wellbutrin or Atomoxetine (a newer one), which are sometimes used with ADHD.
I have inattentive ADHD, and I'm currently taking Wellbutrin (bupropion), which is working pretty well. I've seen mixed reviews of it online: some people like it and others don't. But this study found that it tended to work well in children:

Good luck with dealing with all these problems!
