Falsified medical records

Hello,

I just want to know if I'm the only one with falsified medical records on here? WHY would they even think to do that? Then everyone believes what the NP, RN, CNA, MD, etc. stated and don't want to hear or believe what I say. I never literally threatened any small children, and I should've never been locked up to begin with. Plus, this one family member who blows up what is a minor problem and always gaslighting the prescriber a problem which doesn't exist or is VERY MINOR.

If I did anything, it's because of overconsumption of caffeinated coffee, which makes me get only 3 hours of sleep. It also makes me irritable and suspicious. Plus, I never ran inside anyone's house.

Stupid *mod edit* docs, trying to make me look more crazy. When I was never crazy.

Also, a word of advice to people who write graffiti - you could get locked up in a mental hospital for that.

I have known this to happen before. You are not the first person to have inaccurate information recorded. Oftentimes it is a misinterpretation done in good faith and nothing sinister.

On most occasions there is an innocent answer. It is very uncommon for doctors to falsify records but it does happen, and there have been cases published in the media. When doctors do falsify records it is usually to cover up medical negligence, often by destroying or altering records. I would be cautious making formal allegations until you know for certain what is going on here. You do not want to damage professional relationships.

If a family member is communicating false, inaccurate or very exaggerated information they might have a mental illness or even a toxic or abusive personality. Or they could have made a genuine mistake. Or maybe the doctor in good faith misinterpreted them.

You state you are not being believed. This could mean you are indeed wrong. If you are indeed correct that a family member is communicating false information your only option here is to involve other family members, work colleagues, or witnesses who can verify your version of events and confirm the recorded information is incorrect.

If you are able to locate evidence, such as other family and acquaintances to back you up, you should then try to resolve this with the doctor directly or via a complaint procedure within the healthcare system you are utilising.

You can also simply type up a letter with your version of events and challenge the inaccuracies and request it be included in all relevant medical files. If you can get statements from other people to back you up then that is even better. You should have the right to have your version of events included in your files. This might be the most simple thing to do. My advice is to remain calm when writing and try to avoid confrontation. Just deal with the alleged inaccurate information.
