Title: Hear me when I'm well: a multiphase mixed methods study of
Psychiatric Advance Directives in Virginia
Responsible person: Anne Metz, LPC

Dear PSYCHForum Member:

I am conducting a research study on the use of psychiatric advance
directives in Virginia
and how Virginia’s Health Care Decisions Act is
being implemented throughout the state
.

I need your help to make the study valid. Please take a few minutes to
complete this confidential questionnaire. You will not be personally
identified or linked to the answers you provide. Some limited information
on your personal characteristics are requested to help us better interpret
the data and compare the views of different stakeholders.

Your experience is very valuable and important to this effort. The answers
you provide will be used to inform legislators and policymakers who are
interested in giving seriously ill patients a strong voice in their own
health care and treatment decisions.

Thank you very much for your assistance.

Sincerely,

Anne L. Metz
