Hello SubinDoubt, I hope that I'm able to help you with your desire for a contract.
Contracts are actually rarely written beforehand and are generally personalized by each individual Dominant for his or her submissive(s). If you recall, in
50 Shades of Grey Christian Grey wrote the contract initially and simply re-used it for every submissive after his first. As much as
50 Shades of Grey gets wrong about BDSM this is something that's fairly accurate: If you want a contract or your Dominant wants you to have a contract, he or she should be the one to draft it, you both should look over it, make any appropriate edits, and then sign it.
If you're looking for a contract in order to aid you with determining what are/ are not your limits, then you're actually looking more for a BDSM checklist than a contract. Here are some sample checklists I've been able to locate. I'm not sure the quality of any of these (as I don't really use checklists with my submissive), but they seem to be at least fairly comprehensive and will hopefully be helpful.
